On April 22, 2001, at 1108 hours Pacific daylight time, a Cessna 310F, N5834X, was destroyed by impact with mountainous terrain, 3 miles northwest of Carmel Valley, California. The airline transport certificated pilot, the sole occupant, was fatally injured. The personal flight was operated by the owner under 14 CFR Part 91. Instrument meteorological conditions prevailed and the aircraft was operating on an instrument flight rules (IFR) flight plan. The flight originated at the Monterey Peninsula airport at 1105.

According to the Federal Aviation Administration (FAA) Southwest Region Quality Assurance Office, the aircraft arrived at Monterey from Carson City, Nevada, on a visual flight rules (VFR) flight plan. Upon arrival at Monterey, the pilot requested an instrument clearance from VFR conditions on top and executed an instrument approach and landing at Monterey; landing about 1030. According to a preliminary transcript of communications, at 1052:53, the pilot contacted Monterey Air Traffic Control Tower and requested an instrument clearance to VFR conditions on top. The tower issued a clearance to the "Salinas VOR [navigational aide] via the SECA 2 departure, climb to and report reaching VFR conditions on top, if not on top by 6,000 [feet] maintain 6,000 and advise, tops last reported 2,300 [feet], departure frequency will be 133.0, [transponder] squawk will be assigned." The pilot read back the clearance but omitted the assigned altitude in the read-back. The controller restated, "your altitude is 6,000." The pilot acknowledged "6,000." At 1100:05, the aircraft was issued a taxi clearance to runway 10R and was given a transponder code of 4565. At 1104:45, the aircraft was cleared for takeoff on runway 10R. At 1107:00, the tower controller transmitted to the aircraft that he was not receiving the aircraft's transponder. At 1107:20, the tower transmitted "I've got your transponder now, contact departure [control]." At 1107:56, the pilot transmitted "Monterey, twin Cessna 5834 xray is with you (unintelligible) 2,000." At 1108:04, the Monterey Approach controller transmitted "last call to Monterey was that twin Cessna 5834 xray?" At 1108:06, the pilot replied "34 xray." At 1108:10, the controller transmitted "November 34 xray verify you're flying the seca 2 departure, say altitude." At 1008:13, the pilot replied "2,200 climbing." At 1108:14, the controller transmitted "are you canceling IFR and on top now VFR, you're southbound November 34X, radar contact 5 miles east of Monterey." At 1108:28, the pilot replied "we're just about to break out here a little bit." At 1108:30, the controller transmitted "34 xray you're heading directly towards higher terrain turn northbound immediately if you're not VFR." At 1108:40, the controller transmitted "November 34 xray are you on top?" There was no further communication received from the aircraft.

The Federal Aviation Administration (FAA) SECA 2 departure procedure states (in part) "TAKE-OFF RUNWAY 10L/R: Climb via heading 096 [degrees] and intercept the SNS [Salinas navigational aide] R-210 to SNS VORTAC, thence via assigned route, maintain 6,000 [feet]. Expect clearance to filed altitude 5 minutes after departure."

A review of preliminary recorded radar flight path data provided by the FAA's Southwest Region Quality Assurance Office shows that, after departure, the aircraft climbed approximately on a 100-degree (magnetic) heading, until crossing the Salinas VORTAC 210 radial, at which time the aircraft track turned right to approximately a 160-degree heading and the aircraft remained on that heading until radar contact was lost near the accident site.

The accident site was on a south-southwesterly facing mountain slope in the coastal mountain range, 6.6 miles southeast of the Monterey Peninsula airport. The position was at latitude 36 degrees 30.9 minutes north and longitude 121degrees 44.4 minutes west (GPS). The elevation was 1,700 feet mean sea level (msl, GPS). The local mountain slope was about 45 degrees downward to the south and southwest. To the north and northeast of the site, the mountain rose steeply to a ridgeline about 2,000 feet msl. The Carmel Valley, extending east-west to the coast, was visible at the base of the mountain about 1 mile to the south and southwest.

On October 15, 2000, about 1244 hours Pacific daylight time, a Beech 35-33, N1344G, operated by the pilot, descended into the Pacific Ocean off Monterey, California, during climb out on a special visual flight rules clearance. The accident site is in the Monterey Bay National Marine Sanctuary. The accident site was about 2 miles northwest of the Monterey Peninsula Airport, from where the pilot had taken off about 1242. The flight was performed under 14 CFR Part 91. Instrument meteorological conditions existed at the airport. The noninstrument rated private pilot had been issued a special visual flight rules (SVFR) clearance for departure, and had taken off toward the ocean for a personal flight to Sacramento, California. The airplane was destroyed, and the pilot and pilot-rated passenger were fatally injured. On October 18, with the assistance of the United States Coast Guard and local municipal authorities, the main wreckage was recovered from a 60-foot depth. The wreckage was found at global positioning satellite (GPS) coordinates of 36 degrees 36.774 minutes north latitude by 121 degrees 52.260 minutes west longitude. The wreckage was examined immediately following its recovery. In summary, the engine's crankshaft was rotated, compression was felt in the cylinders, and the continuity of the gear and valve train was confirmed. The propeller blades were observed torsionally twisted and bent into an "S" shape. Fuel was observed in the main fuel line leading to the flow divider. The vacuum pump's drive coupling was found intact. The continuity of the flight control system was verified. The throw-over control yoke was found positioned to the left side. A pilot-witness, in another airplane waiting in sequence for takeoff behind the accident pilot, provided a statement to the National Transportation Safety Board investigator. According to this witness, the noninstrument rated pilot occupied the left seat, and the instrument rated passenger (who held a commercial pilot and flight instructor certificate) was in the right seat. The airplane's acceleration and initial climb appeared normal. The witness lost sight of the airplane as it entered the base of the clouds. A jogger, located near the coast reported "I heard the sound of an airplane in a dive (and saw) a plane fly out of the clouds, diving towards the water." The airplane pulled out of the dive. It then rolled sideways while climbing and then "dropped suddenly into what appeared to be a straight downward dive." It descended at what appeared to be a 90-degree angle. At no time did the engine sound as if it were under stress, faltering, or sputtering. It sounded very strong. Numerous other witnesses, who were in boats, also reported observing the airplane during its last few seconds of flight. One boater reported, "I first heard the plane . . . under full throttle." The plane climbed, went inverted, rolled into a dive, and hit the water at full speed. Another boater commented that "it appeared like he came out of the heavy fog . . .". Another boater reported that he was between 300 and 500 yards from the crash site. According to this witness, the airplane "flew out of the low clouds at an angle of approximate 45 to 60 degrees toward the sea." Five boaters reported observing the airplane bank right to "an inverted attitude," and "full throttle" was heard. Three other boaters reported the airplane was attempting to pull out of a fall and "it was out of control." The airplane pulled up, the right wing dipped, the plane rolled and twisted counter-clockwise (cork screwed) and went up, twisting back almost toward the direction from which it came. It was inverted. Then, it appeared to stall and descend straight down into the water. At 1213, Monterey Airport (elevation 254 feet mean sea level (msl)) reported its weather as 5 miles visibility, mist, and a broken ceiling 700 feet above ground level (agl). At 1254, Monterey reported its visibility had decreased to 4 miles in mist. The overcast ceiling had lowered to 500 feet agl.

On July 18, 2000, at 1845 hours Pacific daylight time, a Canadair CL-65, N97325, experienced an autopilot pitch trim master caution light followed by an autopilot pitch trim nose down master caution light, and a subsequent pitch trim malfunction. The flight crew elected to divert to the Fresno-Yosemite International Airport, Fresno, California. The airplane, operated by Mesa Airlines d.b.a. America West Express under 14 CFR Part 121 as America West Express flight 6088, was not damaged. The 3 crew and 20 passengers were not injured. The flight departed the Monterey Peninsula Airport, Monterey, California, at 1820, and was scheduled to terminate at the Phoenix Sky Harbor International Airport, Phoenix, Arizona. Visual meteorological conditions existed for the scheduled domestic passenger flight and an instrument flight rules (IFR) flight plan was filed. The pilot reported that while climbing through FL200 warning lights for the autopilot pitch trim master caution and the autopilot pitch trim nose down master caution illuminated. The autopilot was disengaged in accordance with the airplane flight manual procedures in an attempt to resolve the problem. The flight crew said they had to maintain approximately 2 inches of forward pressure on the flight control column to maintain level flight. They noted that they were able to trim the airplane nose up, but were unable to trim it nose down. Company maintenance personnel replaced the horizontal stabilizer pitch trim actuator and the associated motor control unit. The system then passed diagnostic checks and a functional flight test was conducted with no discrepancies noted. The airplane was subsequently returned to service.

On June 16, 1999, at 2200 hours Pacific daylight time, a Rockwell International 690B, N480K, had a left gear collapse upon landing at Monterey, California. The aircraft sustained substantial damage; however, the commercial pilot and his four passengers were not injured. The aircraft was owned and operated by SB Aircraft Sales of Boulder, Colorado, under the provisions of 14 CFR Part 91 for the cross-country business flight. An instrument flight rules (IFR) flight plan was filed and instrument meteorological conditions prevailed at the airport at the time of the accident. The flight originated at Tri-County Airport near Boulder, Colorado, at 0530 hours local time and was destined for the Monterey Peninsula airport.

NARRATIVE: MR. TEMPLETON RECEIVED CLEARANCE TO LAND AT THE MONTEREY
AIRPORT ON RUNWAY 28L. AT APPROXIMATELY 300' AGL AND APPROXIMATELY 1/3
MILE FROM TOUCHDOWN, HE WAS REQUESTED (BY THE CONTROLLER) TO CHANGE TO
RUNWAY 28R WHICH IS A SHORTER AND NARROWER RUNWAY. THEREFORE, WITH
FLAPS FULLY EXTENDED, HE APPLIED ADDITIONAL POWER TO REALIGN FOR
LANDING. THE AIRCRAFT BEGAN FLARING AND FLOATING AND CAME DOWN HARD AND
BOUNCED. AFTER A SECOND BOUNCE, HE PUSHED THE STICK FORWARD, THE
AIRCRAFT CAME DOWN ON THE NOSE GEAR AND FRACTURED THE NOSE GEAR
CYLINDER SEPARATING THE NOSE WHEEL. THE PROPELLER CONTACTED THE GROUND
AND THE AIRCRAFT SKIDDED TO A COMPLETE STOP ON THE RUNWAY. COPY OF

Narrative
MR. HOOSHANG KARESOONI WAS FATALLY INJURED WHILE SKYDIVING AT THE
MARINA AIRPORT. DEATH WAS THE RESULT OF HIGH SPEED IMPACT WITH THE
GROUND. THE VICTIM HAD CUT AWAY HIS MAIN CHUTE AND FAILED TO DEPLOY HIS
RESERVE CHUTE. RECORDS INDICATED THAT A MR. JOHN GWIAZDA, A SENIOR
PARACHUTE PACKER WORKING AT SKYDIVE MONTEREY BAY, HAD PERFORMED THE
LAST REPACK OF THE VICTIMS RESERVE CHUTE. WITNESSES STATED THAT THEY
OBSERVED THE VICTIM REPACKING HIS MAIN CHUTE AFTER COMPLETING A
PREVIOUS JUMP. INSPECTORS VIDAK AND TORO VISITED WITH MR. GWIAZDA AT
THE SKYDIVE MONTEREY BAY FACILITIES. AN INSPECTION OF THE LOFT AND MR.
GWIAZDA'S RECORDS, WAS SATISFACTORY. ON DECEMBER 17, 1997, INSPECTORS
VIDAK AND MOUNTZ WERE PRESENT FOR THE FORMAL INSPECTION OF MR.
KAREZOONI'S EQUIPMENT. THIS INSPECTION WAS PERFORMED BY MR. ALLEN
SILVER, A CERTIFIED MASTER RIGGER AND OWNER OF SILVER PARACHUTE SALES.
MR. SILVER IS ALSO A DPRE AND RECOGNIZED AS AN EXPERT IN THIS FIELD. NO
DEFECTS WERE NOTED ON THE MAIN CANOPY, THE BRAKES HOWEVER WERE NOT
RELEASED. A CAREFUL AND DETAILED INSPECTION OF THE RESERVE CHUTE
REVEALED NO DEFECTS. ALL COMPONENTS OPERATED PROPERLY AND THE CHUTE WAS
PROPERLY PACKED. THE RESERVE WAS EQIPPED WITH AN AAD. IT IS NOT KNOWN
WHETHER IT WAS ACTIVATED FOR THIS JUMP. HOOSHANG KAZEROONI, USPA

NARRATIVE: PILOT STATED: ON MARCH 9, 1997, I WAS LANDING ON 28R MRY,
WINDS WERE LIGHT BUT A SLIGHT CROSSWIND. AFTER NORMAL 3 POINT
TOUCHDOWN, I FELT MY LEFT WING COMING UP. AFTER LOWERING WING, AIRCRAFT
STARTED A GROUND LOOP TO THE RIGHT, WITH FULL RUDDER AIRCRAFT STILL
HEADED FOR RIGHT SIDE OF RUNWAY. APPLYING LEFT BRAKE DID NOT STOP THE
VEER SO APPLIED BOTH BRAKES TO STOP AIRCRAFT. THE AIRCRAFT LEFT THE
RIGHT SIDE OF RUNWAY AND INTO GRAVEL. AFTER ABOUT 20 FT. OFF RUNWAY,
TAIL CAME UP AND PROP STRUCK THE GROUND AND AIRCRAFT STOPPED WITH COWL
ON THE GROUND. PILOT & PASSENGER EXITED AIRCRAFT, WITH HELP TAIL
LOWERED TO GROUND AND TOWED OFF. DAMAGED LOWER COWL DENTED AND SCRAPED,
PROP TIPS BENT, PROP SPINNER DENTED. INSPECTORS STATEMENT: INSPECTION
OF THE AIRCRAFT ON MARCH 10, 1997, AT MRY VERIFIES PILOTS STATEMENT
CONCERNING DAMAGE TO THE AIRCRAFT. NO OTHER OBSERVABLE DEFECTS OR
DAMAGE WERE NOTED. LOG BOOKS AND CREDENTIALS VERIFIED. AIRWORTHINESS &
REGISTRATION CERTIFICATES VERIFIED VALID. PILOTS WRITTEN STATEMENT
OBTAINED. NO PILOT DEVIATION AND NO 8020.11 GENERATED. THIS TERMINATES

NARRATIVE: PILOT STATED: ON DOWNWIND AT MRY, RWY 28R, THE AIRCRAFT
THROTTLE WAS REDUCED TO DECREASE AIRSPEED. AS THE THROTTLE WAS PULLED
BACK THE ENGINE IMMEDIATELY BEGAN TO RUN ROUGH. AN EMERGENCY WAS
DECLARED AND THE AIRCRAFT WAS LANDED UNEVENTFULLY. JUST AFTER
TOUCH-DOWN, THE ENGINE QUIT AND THE AIRCRAFT WAS TAXIED OFF THE RUNWAY
UNDER ITS OWN MOMENTUM. THE CAUSE OF THE ROUGH RUNNING ENGINE IS UNDER
INVESTIGATION AND WHEN THE CAUSE IS DETERMINED, BY MAINTENANCE

NARRATIVE: ON JUNE 3, 1996, A BEECH 58P, N580TC, ENROUTE FROM SAN LUIS
OBISPO (SBP) TO SAN JOSE (SJC) LOST ENGINE POWER TO THE RIGHT ENGINE.
THE PILOT, MR. JOSE PEREZ, REPORTED TO MONTEREY (MRY) APPROACH AT
APPROXIMATELY 2110 THAT HE WAS HAVING ENGINE PROBLEMS AND WAS INTENDING
TO LAND AT SALINAS (SNS), CA. WHILE IN APPROACH TO LAND AT SNS THE
RIGHT ENGINE SEIZED TO OPERATE. AN UNEVENTFUL LANDING WAS MADE. MR.
PEREZ STATED THAT AT APPROXIMATELY 2100 THE RIGHT ENGINE STARTED TO RUN
ROUGH. HE APPLIED BOOST PUMP PRESSURE AND THE AIRCRAFT CLEARED UP FOR
APPROXIMATELY 5 MINUTES. WHEN THE ENGINE DEVELOPED THE PROBLEM AGAIN HE
CHECKED HIS INSTRUMENTS AND NOTICED OIL PRESSURE DROPPING AND OIL
TEMPERATURE RISING. HE DECIDED TO DIVERT TO SNS. A VISUAL INSPECTION OF
THE AIRCRAFT ENGINE, A TELEDYNE-CONTINENTAL TSIO-520L, WAS ACCOMPLISHED
ON JUNE 6, 1996. THE ENGINE HAD ALREADY BEEN REMOVED FROM THE AIRCRAFT.
PRIOR TO REMOVAL, THE MECHANIC AT GOLD COAST AVIATION STATED THAT THE
PROPELLER COULD NOT BE TURNED AND THE ENGINE HAD SUFFICIENT OIL.

EXTERNAL INSPECTION DID NOT REVEAL ANY VISUAL EVIDENCE OF DAMAGE, OIL
LEAKS, ETC. INSPECTION OF AIRCRAFT ENGINE RECORDS REVEALED THAT THIS
ENGINE WAS OVERHAULED 71.4 HOURS PRIOR. ENGINE TAGGED FOR DISASSEMBLY
INSPECTION BY FAA AND SUBMISSION OF FOLLOW UP-REPORT. NO FAA 8020-11

OWNED AND OPERATED BY MC MAHANS FURNITURE CO. OF LAS VEGAS, NEVADA, WAS
INVOLVED IN A DECLARATION OF AN EMERGENCY DURING A BUSINESS FLIGHT FROM
CARLSBAD, CALIFORNIA, TO MONTEREY, CALIFORNIA. VISUAL METEROLOGICAL
CONDITIONS PREVAILED AND AN IFR FLIGHT PLAN WAS FILED. THE AIRCRAFT
RECEIVED NO KNOWN DAMAGE AND LANDED AT MONTEREY PENINSULA AIRPORT WITH
NO FURTHER INCIDENT. THE PILOT IN COMMAND AND CO-PILOT WERE NOT
INJURED. THERE WERE NO REPORTS OF ANY PASSENGERS ONBOARD THE AIRCRAFT
DURING THIS FLIGHT. THE PILOT IN COMMAND AND SECOND IN COMMAND REFUSED
TO IDENTIFY THEMSELVES AS THEY FELT THEIR IDENTITY WOULD BE ASSOCIATED
WITH THE INCIDENT RECORD AND WOULD ADVERSELY AFFECT THEIR PENDING
AIRLINE EMPLOYMENT PROSPECTS. THE PILOT IN COMMAND DID STATE THAT

ONE FIRE EXTINGUISHING BOTTLE WAS DISCHARGED WITH NO EFFECT ON THE FIRE
WARNING LIGHT. 30 SECONDS LATER THE SECOND FIRE BOTTLE WAS DISCHARGED
BUT THE FIRE WARNING LIGHT STAYED ILUMINATED. THE CREW DECLARED AN
EMERGENCY TO MONTEREY APPROACH CONTROL (TRACON). THE MONTEREY AIRPORT
FIRE DEPARTMENT WAS DISPATCHED TO THE RUNWAY 28L AND A VISUAL
INSPECTION OF THE CESSNA CITATION WAS CONDUCTED AS THE AIRCRAFT TAXIED
OFF THE RUNWAY. NO FLAMES OR SMOKE NEAR THE RIGHT ENGINE WAS REPORTED.
A MECHANIC CHECKED THE ENGINE AFTER THE CREW PARKED AND PERFORMED THE
SHUTDOWN AND FOUND A MECHANICAL PROBLEM WITH A "FIRE LOOP" WIRE WHICH
MAY HAVE CAUSED THE FIRE WARNING LIGHT TO ILLUMINATE WHEN IN FACT NO

NARRATIVE: ON AUGUST 3, 1995, AT 14:00 PDT, A CESSNA 182, OWNED BY
ROBERT WOLFE WAS INVOLVED IN A LANDING INCIDENT DURING A PERSONAL
FLIGHT. VISUAL METEOROLOGICAL CONDITIONS PREVAILED AND A VFR FLIGHT
PLAN WAS FILED. THE AIRCRAFT RECEIVED MINOR DAMAGE AND THERE WERE NO
INJURIES TO THE PILOT AND PASSENGER. THE PILOT HELD A PRIVATE PILOT
CERTIFICATE. THE FLIGHT ORIGINATED AT VAN NUYS, CA, ON AUGUST 3, 1995,
AT 11:30 PDT. THE PILOT STATED, THAT WHILE BRAKING AFTER LANDING, ON
RUNWAY 28L, THE LEFT BRAKE FAILED. THIS CAUSED THE AIRCRAFT TO SWERVE
SHARPLY TO THE RIGHT. THE AIRCRAFT RAN OFF THE RIGHT EDGE OF THE RUNWAY
AND STRUCK A TAXIWAY SIGN. THE TIRE AND WHEEL WERE DAMAGED BY THE
COLLISION WITH THE SIGN. A DEL MONTE AVIATION MECHANIC FOUND NO FLUID

NARRATIVE: THE PILOT SAID WHEN HE PLACED THE LANDING GEAR SWITCH IN THE
DOWN POSITION, THE LANDING GEAR CIRCUIT BREAKER POPPED. HE SAID HE
RESET THE CIRCUIT BREAKER AND IT POPPED AGAIN. HE SAID HE THEN
REQUESTED A GO-AROUND TO ALLOW ATC TO OBSERVE THE GEAR. HE SAID HE THEN
LOWERED THE GEAR MANUALLY ACCORDING TO THE AIRCRAFT FLIGHT MANUAL
CHECKLIST. AFTER EXECUTING THE MANUAL EXTENSION PROCEDURE, THE PILOT
SAID HE OBSERVED THE FLOOR INDICATOR THAT INDICATED THE GEAR WAS DOWN.
HOWEVER, THE GEAR UNSAFE ANNUNCIATOR REMAINED ILLUMINATED. HE SAID ATC
OBSERVED THAT THE GEAR APPEARED TO BE DOWN.. THE PILOT SAID THE LANDING
FELT NORMAL UNTIL THE AIRCRAFT STARTED TO SLIDE SIDEWAYS. HE SAID HE
THEN SHUT OFF FUEL TO SHUT DOWN THE ENGINE. HE SAID THE FIRE DEPARTMENT
INFORMED HIM THAT THE RIGHT MAIN GEAR APPEARED TO FOLD UNDER THE
AIRCRAFT FIRST WITH THE REMAINING GEAR FOLLOWING. NO FURTHER ACTION IS
REQUIRED AND THIS CLOSES THE REPORT.

On October 11, 1999, about 1028 hours Pacific daylight time, a Pitts S2, N50AL, sustained substantial damage during a loss of control on the landing rollout at the municipal airport in Salinas, California. Pitts S2B LLC operated the airplane under the provisions of 14 CFR Part 91. The private pilot was not injured. The local personal flight departed Salinas at an undetermined time. Visual meteorological conditions prevailed and no flight plan was filed. The pilot told a Federal Aviation Administration inspector he was practicing touch and go landings. On the third attempt the airplane bounced, and during the recovery attempt, the wing and propeller were damaged when they contacted the ground.

On June 16, 1999, at 2200 hours Pacific daylight time, a Rockwell International 690B, N480K, had a left gear collapse upon landing at Monterey, California. The aircraft sustained substantial damage; however, the commercial pilot and his four passengers were not injured. The aircraft was owned and operated by SB Aircraft Sales of Boulder, Colorado, under the provisions of 14 CFR Part 91 for the cross-country business flight. An instrument flight rules (IFR) flight plan was filed and instrument meteorological conditions prevailed at the airport at the time of the accident. The flight originated at Tri-County Airport near Boulder, Colorado, at 0530 hours local time and was destined for the Monterey Peninsula airport.

On April 14, 1999, about 1800 hours Pacific daylight time, a Piper PA-31, N141CM, experienced a loss of engine power and force landed the aircraft in the Pacific Ocean approximately 15 to 20 miles off the coast of Monterey, California. The aircraft sustained substantial damage and subsequently sank. The commercial pilot sustained minor injuries. The aircraft was operated by Tokyo International Trading America Inc., Walnut, California, and the flight was conducted under 14 CFR Part 91 as a ferry flight. The aircraft departed the Honolulu, Hawaii, airport, about 0448, and was en route to Monterey, California. Visual meteorological conditions prevailed at the accident site. A VFR flight plan was filed. The Oakland Air Traffic Control Center (ARTCC) reported that when the pilot was about 30 miles offshore, he informed them that he might have to ditch the aircraft in the ocean due to fuel exhaustion. The pilot subsequently ditched the aircraft in the Pacific Ocean. He evacuated the aircraft, activated a life raft, and was rescued by the United States Coast Guard approximately 30 minutes later.

The aircraft departed Monterey Airport on a local flight and was reported overdue on the day of the flight. It was subsequently found approximately 18 miles southeast of the airport where it had impacted mountainous terrain. The pilot and one passenger were fatally injured and the aircraft was consumed by a postimpact fire.

During an air show, the pilot made several uneventful flybys, then landed near the beginning of the runway. During rollout, the pilot noted a hydraulic brake system failure. Unable to stop with only unboosted brakes, the airplane overran the departure end of the 5,005 foot runway and collided with a fence and rough terrain. Investigation revealed the engine driven #3 system auxiliary hydraulic boost pump had failed internally. Metal particles were found in its screen.
Probable Cause
an internal failure of a hydraulic pump, which resulted in contamination in the hydraulic system and subsequent loss of the braking system. Factors related to the accident included the proximity of a fence and rough/uneven terrain adjacent to the runway.

WHEN THE BEECH 95 CREW RPRTD THE BAE-146 WAS IN SIGHT, THEY WERE CLRD TO FOLLOW IT. A WITNESS, WHO WAS WATCHING THE BAE-146 LAND, STATED THAT WHEN HIS ATTENTION WAS FOCUSED ON THE PLANE (BEECH 95) SOME DISTANCE BEHIND THE COMMERCIAL PLANE (BAE-146), IT WAS IN A STEEP NOSE DOWN DSCNT TOWARD THE GND & IN A RGT SPIN. THE BEECH 95 CRASHED IN APRX A 125 DEG NOSE DOWN ATTITUDE. NO PREIMPACT PART FAILURE OR MALFUNCTION WAS FOUND. HOWEVER, THE ACFT WAS FND CONFIGURED FOR A PSBL SIMULATED ENG-OUT APCH; I.E., THE RUDDER TRIM TAB & INDICATOR WERE FND WITH ABT 20 TO 22 DEG DEFLECTION, THE LEFT & RGT TACH INDICATED 1900 & 2700 RPM, RESPECTIVELY, & THE FLAP & FLAP ACTUATORS WERE FND IN A RETRACTED PSN. THE IN-FLT SEPN BTW THE BEECH 95 & BAE-146 WAS NOT DETERMINED. TWR PSNL DIDN'T PROVIDE A WAKE TURBC ADZY.

THE AIRCRAFT WAS RADAR VECTORED TO THE FINAL APPROACH FOR AN ILS LANDING. AS THE AIRCRAFT APPROACHED THE FINAL APPROACH FIX THE CONTROLLER CLEARED THE AIRCRAFT FOR THE APPROACH AND INSTRUCTED THE PILOT TO CONTACT THE TOWER. THE PILOT ACKNOWLEDGED THE INSTRUCTION THEN STATED THAT HE WAS STARTING A GO-AROUND. APPROXIMATELY 30 SECONDS LATER THERE WAS AN UNINTELLIGIBLE RADIO TRANSMISSION FROM THE AIRCRAFT. THIS TRANSMISSION WAS FOLLOWED BY A CALL FROM THE PILOT STATING, "SEVEN NINER THREE IN TROUBLE". THIS TRANSMISSION WAS FOLLOWED 16 SECONDS LATER BY, "I HAVE I HAVE AN EMERGENCY BACK HERE". THE CONTROLLER HAD NOTICED AN ASCENT 400 FEET ABOVE THE ASSIGNED ALTITUDE THEN THE RADAR WENT INTO COAST. A WITNESS IN A BOAT IN THE VICINITY OF THE ACCIDENT OBSERVED THE AIRCRAFT DIVE OUT OF A CLOUD AND COLLIDE WITH THE WATER.

THE FLT ARRIVED IN MONTEREY, CA, THAT AFTERNOON. AFTER DINNER THE FLT WAS RETURNING TO THE POINT OF ORIGINAL DEPARTURE. WHEN THE PLT CONTACTED THE TWR FOR TAXI INSTRUCTION HE WAS INFORMED THAT MONTEREY WX WAS BELOW VFR MINIMUMS. AFTER A DISCUSSION OF WX CONDITIONS IN THE VICINITY AND THE NEARBY MOUNTAINS, THE CONTROLLER ISSUED A SPECIAL VFR CLEARANCE OUT OF THE CONTROL ZONE. AT 2051 PDT THE ACFT WAS CLEARED FOR TAKEOFF. AT 2054 WHEN ASKED BY THE CONTROLLER IF THE HIGHWAY WAS IN SIGHT THE PLT RESPONDED "ROGER." THE CONTROLLER INSTRUCTED THE PLT TO STAY OVER THE HIGHWAY AND TO STANDBY ON TWR FREQ. AT 2055 THE CONTROLLER CALLED THE ACFT TO TURN LEFT WARNING OF RISING TERRAIN. THE ACFT WAS OBSERVED ON RADAR TO CLIMB IN A RIGHT TURN FROM 900 TO 1500 FEET BEFORE RADAR CONTACT WAS LOST ABOUT 4.5 MILES EAST OF THE ARPT. THE ACFT IMPACTED THE 30 DEGREE UPSLOPING TERRAIN IN A NOSE DOWN INVERTED ATTITUDE AND WAS CONSUMED BY POST CRASH FIRE. ELEVATION AT THE ARPT AND ACCIDENT SITE WERE 244 AND 744 FT MSL RESPECTIVELY.

THE PLT STATED THAT COMMUNICATION AND NAVIGATION PROBLEMS WERE EXPERIENCED DURING THE FLT. HE ALSO STATED HE CHANGED FUEL TANKS EVERY 1/2 HR DURING THE FLT. A MISSED APCH WAS PERFORMED DURING THE FIRST ILS ATTEMPT BECAUSE THE PLT COULD NOT "POSITIVELY" IDENTIFY THE RWY THRESHOLD. ON THE 2ND APCH, AFTER OUTER MARKER PASSAGE, THE PLT NOTICED HE WAS BELOW GLIDESLOPE AND NECESSARY ADJUSTMENTS WERE MADE. HE CONTINUED TO STATE THAT AFTER TIME PASSAGE HE NOTICED FULL DOWN GLIDESLOPE DEFLECTION AND MIDDLE MARKER PASSAGE HAD NOT OCCURRED. SHORTLY THEREAFTER, THE ACFT IMPACTED TERRAIN. THE INSTRUMENT PANEL AND RADIO PACK WERE REMOVED FROM THE ACFT BEFORE INVESTIGATORS ARRIVED. THE RT FUEL TANK WAS FOUND EMPTY AND THE LT WAS NEARLY FULL. NO MECHANICAL FAILURE/MALFUNCTION WAS FOUND. RECORDS INDICATE LAST TRANS CHECK WAS 5/9/77, LAST STATIC SYSTEM/ALTIMETER CHECK WAS 5/12/77. LAST VOR TEST COULD NOT BE DETERMINED.

THE ONLY NUT PRESENT WAS LOOSE BUT RETAINED THE #1 CYL EXHAUST TUBE. THE #2 CYL EXHAUST TUBE, THOUGH NOT SECURED, WAS IN PLACE. THE #3 & #4 CYL EXHAUST TUBES WERE UNRESTRAINED & HAD DROPPED DOWN, ALLOWING EXHAUST GASES TO ESCAPE. HEAT DAMAGE ALONG THE LEFT SIDE OF THE ENG WAS MUCH WORSE THAT ON THE RIGHT SIDE, & THE FUEL PUMP FUEL LINE WAS BURNED ENOUGH TO ALLOW FUEL UNDER PRESSURE TO ESCAPE INTO THE ENG COMPARTMENT.

DURING THE LANDING ABOUT 2 MILES SHORT OF THE ARPT THE ACFT NOSED OVER IN A SOFT MUDDY PLOWED FIELD.

THE FUEL STRAINER DRAIN FITTING BROKE OFF AND FUEL ESCAPED. A FIRE STARTED AND BURNED A HOLE IN THE BOTTOM OF THE ACFT.

ALL 4 OCCUPANTS WERE ABLE TO EXIT THRU THE AIRSTAIR DOOR AND RECEIVED ONLY MINOR INJURIES. THE ACFT SANK AFTER ABOUT 5 MINUTES. IT WAS NOT RECOVERED FROM THE 27 FT DEEP WATER. TWO OCCUPANTS SWAM ASHORE AND 2 WERE RESCUED BY THE COAST GUARD. THE LINE ATTENDANT WHO SERVICED THE ACFT MISTOOK IT FOR A KING AIR. THE PILOT REQUESTED THAT ALL 4 TANKS BE TOPPED OFF. THE LINE ATTENDANT ADDED 110 GALLONS OF JET A FUEL.

TWO MINUTES AFTER TAKEOFF THE PILOT WAS TOLD TO CONTACT DEPARTURE; THE PILOT ACKNOWLEDGED. THIS WAS THE LAST RECORDED RADIO CONTACT. THE AIRPLANE COLLIDED WITH A HILL OBSCURED BY GROUND FOG ABOUT 3 MI EAST OF THE AIRPORT. THE MAN HIRED TO FLY THE AIRPLANE DID NOT POSSESS AN AIRMAN CERTIFICATE; HIS CERTIFICATE WAS REVOKED 2 YEARS PRIOR TO THE ACCIDENT. THE OWNER HAD OBTAINED HIS PRIVATE CERTIFICATE FOR AIRPLANE SINGLE-ENGINE LAND ABOUT 1 MONTH PRIOR TO THE ACCIDENT, AND HAD NOT RECEIVED ANY MULTI-ENGINE INSTRUCTION.

THE OWNER/PILOT'S POOR JUDGEMENT IN ATTEMPTING AN OPERATION BEYOND HIS EXPERIENCE AND ABILITY, WHICH RESULTED IN HIS FAILURE TO ATTAIN AN ADEQUATE CLIMB PROFILE NECESSARY FOR TERRAIN CLEARANCE.
